AFFIDAVIT FOR CHANGE OF NAME

I, [OLD NAME], S/o [FATHER/HUSBAND NAME], aged about [AGE] years, residing at [ADDRESS], do hereby solemnly affirm and declare as follows:

1. That my name in all my previous official documents, including educational records, Aadhaar, and PAN, has been recorded as "[OLD NAME]".

2. That I have decided to change my name from "[OLD NAME]" to "[NEW NAME]" out of personal choice and voluntary preference.

3. That henceforth, I shall at all times and on all occasions, in all records, deeds, proceedings, dealings, and transactions, both private and public, be known, addressed, and referred to solely as "[NEW NAME]".

4. That (1) [OLD NAME] and (2) [NEW NAME] represent one and the same living person, that is myself.

5. That this affidavit is executed for the purpose of getting my new name recorded in all official government records, passport, educational certificates, banking accounts, and for publication in the Official Gazette and local newspapers.
